The incumbent Physical Therapist is assigned responsibility for serving as the subject matter expert in Geriatric Physical Therapy.
Incumbent's primary function is direct patient/client care.
Functions:
Applies knowledge of the anatomical, biological, physical, cognitive, psychological, and psychosocial changes that occur with aging clients, including disease and disability, and the effects of inactivity on these changes.
Develops, coordinates, and implements highly specialized treatment programs within his/her assigned clinic, based on best practices for patient/client management, to relieve pain, restore or enhance performance, and maximize independence in functional abilities.
Develops a plan of care for the physical therapy management of patients/clients with complex medical profiles (e.g., frailty, heart failure, mechanical ventilation dependency, multiple chronic health conditions, dementia, malignant neoplasm, multiple traumatic injuries).
Develops evidence-based and patient-centered physical therapy interventions for conditions commonly encountered with older adults, utilizing enablement- disablement frameworks, emphasizing functional movement, and considering principles of optimal aging across physiological systems.
Adapts the plan of care to address disabling psychosocial factors (e.g., depression, learned helplessness, anxiety, fear of falling).
Evaluates clinical situations where standard treatment recommendations, based on best evidence, should be modified with regard to older adults' preferences & treatment/care goals, life expectancy, co-morbid conditions, and/or functional status.
Recognizes the need for continuity of treatment and communication across the spectrum of services and during transitions between care settings, utilizing information technology where appropriate and available.
Provides information to older adults and their caregivers about the continuum of long term care services and supports - such as community resources, home care, assisted living facilities, hospitals, nursing facilities, sub-acute care facilities, and hospice care.
Educational activities:
The incumbent enhances his / her knowledge and skill in geriatrics by participating in continuing professional development through but not limited to advanced degrees, certification, continuing education seminars, self-studies, familiarity with current literature on evidence based geriatric physical practices.
Participates in, provides and attends Physical Therapy Department in-services as well as hospital-wide in-services. Completes all required VA annual mandatory training. Attends mandatory in-service training as required by VA, PM&RS and PT policies.
Has a responsibility to the profession and this facility to assist with the instruction of Physical Therapy and Physical Therapy Assistant students assigned to this facility for clinical affiliations, and possess skills commensurate with that responsibility.
Provides guidance/direction and education to volunteers in the Physical Therapy Clinics.
May serve as a resource person to the community in matters relating to Physical Therapy, the delivery of health care, and the needs of the veteran/military population.
